Under current federal guidelines, Pine Lake Preparatory is not required to automatically mail Form 1095-C to all employees. Your form is available upon request.
You may download your Form 1095-C anytime through our benefits portal:

If you would prefer a paper copy mailed to you, please contact us and we will send it within 30 days of your request.
Former employees and individuals who retired in 2025 will automatically receive their Form 1095-C by mail.
